Lighthouse understands that people with learning disabilities are more likely to experience mental health problems which may increase as they get older. We also recognise that peoples’ physical health may decline with age.
We provide specialist older people's services which help to meet these needs.
Calling upon the skills and discipline of our multi-disciplinary team we are committed to tackling the adverse impact that a learning disability, coupled with age-related physical problems, can have on the individual. This involves management of complex physical healthcare conditions and assessment and management of dementia.
Through regular assessment and development of person-centred care plans, individuals are supported on their care pathway and encouraged to increase their levels of independence, despite their increasing years.
