CARE SERVICES

We provide specialised person-centred care planning services to people with challenging behaviour and complex needs.

CARE FACILITIES

From low secure hospitals through to community-based care homes, we offer people a structured route along their care pathway.

Learning Disabilities

Our services range from hospital provision through to two-bedroom supported bungalows for men and women with learning disabilities.

In addition to their learning disability, many of our service users also have a background of challenging behaviour and mental health needs, with some also suffering from personality disorder or an Autistic Spectrum Condition.

The treatment programme for individuals with a learning disability begins with a comprehensive multi-disciplinary team assessment. An individual treatment plan helps facilitate the care pathway for service users. These plans are person-centred and may include behavioural therapy, psychotherapy, specialist offender treatment programmes (for example sex offending treatment), substance misuse and educational and vocational training. Each person’s individual care pathway is continually monitored and appraised with regular reviews of progress and outcomes, in partnership with the person themselves, commissioning agencies, family members, independent advocates and others.

Within our learning disability services we also provide specialist female-only environments designed to meet the individual needs of women.
