A specialist hospital registered with the Care Quality Commission, Field House, situated in Alfreton, Derbyshire, provides services for older people with a primary diagnosis of learning disability.
Our service users often have a background of challenging behaviour and mental health needs and are likely to require a specialist placement because of their age, or problems associated with the ageing process.
Typically, our service users come from a secure setting with a forensic history and whose needs have become more complex due to getting older and having physical health problems that need addressing. This involves management of complex physical healthcare conditions and assessment and management of dementia.
Through regular assessment and development of person-centred care plans, individuals are supported on their care pathway and are encouraged to increase their levels of independence, despite their increasing years
